MEIDC’s COMMITMENT TO YOUR PRIVACY

Modern Emergency ID (MEIDC) believes that your personal information is just as important as our own. MEIDC places significant value on personal privacy and security.

PERSONAL INFORMATION SHARING. We realize that our customers enter and retain non-public personal and health information (“NPI”) on devices where the MEIDC Application (“App”) is installed, and it is our policy to secure this information on your device in a confidential manner but also to make it available to first responders and health care providers. Please do not enter or retain information that you are not comfortable sharing with first responders and health care providers.

PERSONAL AND HEALTH INFORMATION ON YOUR DEVICE. The MEDIC App allows you to store non-public personal and health information (“NPI”) on your personal device. That NPI is retained in the personal device where you installed the MEIDC App. Your NPI is not stored in a centralized database maintained by MEIDC, nor is it transmitted to or accessible to MEIDC from the App except in the course of generating physical identification cards. It will not be shared by MEIDC with any third party, except as provided by MEIDC’s policies and procedures.

The NPI and other information that is stored on your personal device can be read, and is intended to be read, by others such as first responders and health care providers. Your NPI and other information will be electronically transmitted from the barcode or manually entered into the ePCR software and devices used by first responders, good samaritans and health care providers. This data may also be shared as part of your electronic health record during an emergency response, hospital visit or other medical care.

Any information that you voluntarily enter into the MEIDC App stays on your personal device. The information entered into the MEIDC App may be accessible to anyone in possession of your mobile device. You are responsible for the control of your device and the privacy settings on your personal device. You may also choose to store a copy of the encrypted data in your own personal cloud data storage for future use. You are also responsible for the privacy and security of your own personal cloud data storage.

Any information entered into the App or provided to MEIDC for printing of a physical identification card is done so voluntarily. The purpose of providing information is so that MEIDC can make your information readily available during an emergency response, hospital visit or other medical care. Therefore, if you choose not to provide any of the requested information MEIDC will be unable to provide first responders and other healthcare professionals access to the omitted information. Notwithstanding this fact, MEIDC does not intend or represent that any personal or health information, whether provided or not, will affect your treatment, payment, or eligibility for medical benefits or services. Any information provided may be disclosed to state or federal agencies as required by law. You have the right to review the records MEIDC keeps about you during normal business hours.

MEIDC is not considered a covered entity nor a clearing house by the US Department of Health and Human Services under the Health Insurance Portability and Accountability Act of 1996. MEIDC does, however, strive to maintain the confidentiality of your information and protect the information you have volunteered into the MEIDC App while allowing the flow of health information needed to provide and promote high quality of health care and to protect the public’s health and well being.

GENERATING PHYSICAL IDENTIFICATION CARDS. If you order a physical identification card from MEIDC, certain information will be encrypted and transmitted to MEIDC printer and used in the course of generating your identification card. The identification card information will be used and retained for a brief period by MEIDC. It will then be purged in accordance with MEIDC’s identification card information policy which may change from time to time. SHARING INFORMATION WITH THIRD PARTIES. Please be assured we will not sell your name, address, email address or other NPI to third parties, nor do we share your NPI with any third parties to market any product or service, except as required to share your information as necessary to complete the performance of your purchase or licensing of any product or App from us, or in other ways, usually under certain conditions. For example, we may share your NPI if state or federal law requires it for, or in event of legal process compelling the disclosure of information.

PROMOTIONAL COMMUNICATIONS.   MEIDC may, from time to time, send e-mail messages to you containing promotions for new MEIDC products or reminders to order a new card if your personal information has changed. We will not release your email to any third party or send you any communications on behalf of any third party. If you no longer wish to receive these emails, simply click the unsubscribe link that is contained in each email or turn off the option in the App.

WHAT DOES MEIDC DO TO PROTECT MY DATA? MEIDC is committed to providing the highest level of security and privacy regarding the encryption of customer’s NPI both in the App and in generating physical identification cards. MEIDC uses state-of-the-art encryption of all data stored on the user’s device via Advanced Encryption Standard – 256 (AES-256). MEIDC secures data transmitted between the App and our servers (via sFTP). MEIDC internal systems are protected by a SonicWALL state-of-the-art firewall and additional layers of physical security to ensure privacy at all stages of production, an FIPS-Approved limited physical access to facilities and printed materials on a need-to-know, access basis only and internal 24/7 facility video monitoring. We use custom printer software that has the only encryption key to unscramble the transmitted data as an extra layer of customer data security. The printing ribbon is also destroyed after use. We take the security of your data very seriously and are fully committed to protecting your personal information. Please be aware, however, that no security measures can guarantee perfect security. We reserve the right to modify this policy, and will provide you with a new policy statement if we make material changes to our privacy or security practices.

RELEASE AND WAIVER OF PRIVACY RIGHTS. By executing the identification verification and privacy waiver (“IDVPW”) within the MEIDC App, each user acknowledges, understands and agrees that:
1. The user is authorizing the use and transmittal of my non-public personal and health information, which may include information relating to alcohol and drug abuse, mental health treatment, and confidential HIV related information to any and all emergency responders, good samaritans, and health care providers who have access to such information via the MEIDC App.
2. The user is authorizing redisclosure of such information to any and all other emergency responders, good samaritans and health care providers without my further authorization, even though such redisclosure may not be protected by federal or state law.
3. The user may revoke this authorization except to the extent that action has already been taken based on the user’s prior authorization.
4. The user has the right to revoke this authorization at any time by notifying MEIDC or any other concerned health care provider or associate in writing.
5. The user is signing this authorization and waiver voluntarily.

IDVPW IDENTIFICATION VERIFICATION. By executing the identification verification and privacy waiver (“IDVPW”) within the MEIDC App, the user represents that that the signature used on the MEIDC App IDVPW matches and does not significantly differ from the signature found on the user’s current government issued identification.
